Nurses PRACTICE NURSESOur Practice Nurses provide nursing care to patients at the practice. They deal with prevention and treatment and are available for general advice. They run their own clinics for Diabetes, Asthma and Chronic Airways Disease, Coronary Heart Disease, Stroke, Child Immunisations and Travel. HEALTH CARE ASSISTANT We have two Health Care Assistants who are trained to take blood (phlebotomy). They run short clinics each morning to take blood when requested by your doctor or nurse. They are also able to test urine, take ECG heart tracings and to monitor blood pressure and weight. DISTRICT NURSES District Nurses provide skilled nursing care to patients in their own homes. We are fortunate in having an excellent district nursing team attached to the practice. Anyone can be referred to a district nurse for a nursing assessment. Care will then be planned and implemented as needed or you may be referred on to other more appropriate services. District Nurses provide care for the chronic sick, acute illnesses, wound care, leg ulcer management and prevention, early discharges from hospital, palliative and terminal care, counselling and support for patients and carers. Due to budgetary constraints at the Buckinghamshire Primary Care Trust (PCT), the district nursing team may not be able to do everything they have previously done for our patients. If they are not allowed to assist then they will discuss alternative sources of help with you. They have their own telephone number with an answering machine for when they are out of the office. Please contact them on 01844 292227 HEALTH VISITORS We have a Health Visitor team attached to the practice who are available to give advice and support on any matters affecting health, particularly for expectant mothers, families with young children and the elderly. As well as monitoring and advising on the growth and development of young children, they also run post-natal groups to support mothers and teach baby massage. Due to budgetary constraints at the Buckinghamshire Primary Care Trust (PCT), the health visiting team may not be able to do everything they have previously done for our patients. If they are not allowed to assist then they will discuss alternative sources of help with you. The Health Visitor team has the following members: Alex, Ali H., Ali M., Gillian and Karen. MIDWIFE The Midwife is attached to the practice and is available to provide maternity services in practice. If you are pregnant, you can collect a maternity pack from reception. As well as providing useful information for the early stages of pregnancy, there is a form inside which we would ask you to complete and return to reception. This informs the midwife of your pregnancy and allows her to contact you to arrange the necessary tests and first appointment. |
